Returning Student-Athlete Clearance Information

(For Student-Athletes that played a sport at Keuka College in 2024-25)

New Student-Athlete Forms

(For Student-Athletes that DID NOT play a sport at Keuka College in 2024-25)

  1. Intro Letter for New Student-Athletes
  2. Keuka Student Health Portal Instructions (Due by July 15)
  3. A Physical must be completed by your Physician (per NCAA) - Physical Form (Updated for 2025-26 academic year)
    1. Esports does not require a physical
    2. Cheerleading must have their physical dated after Sept. 1, 2024
    3. Winter and Spring sports must have their physical dated after Feb. 25, 2025
    4. Fall sports must have their physical dated after Feb. 15, 2025

We will continue to monitor the news and medical developments and we may follow up at a later date to request further information or documentation if needed based on recommendations from the CDC, NYS, or Yates County Health Department.
